Clean up the Safety Concern section of the Participant Survey form GH2731
This request is two part
1.) Modify the label and description of the Y/N Safety Concern question
Label: "Were there any safety incidents or other concerns to report?"
Description: [NO CHANGE]
2.) Add a text field to the Participant Survey to describe their safety concern.
Label: "Please describe your concern"
Description: "Provide a short description of your safety concern"
Content Type: Text field
Business Rules: Like #1 above, if content is entered into this text field, send safety report to Safety@mountaineers.org
User Story: As a trip participant, when I enter a description of my safety concern and click Submit to the form, send a notification to the Safety Committee so that they may act to follow up.
We added the text field asking for a brief summary.
Sending this to the Safety Committee for either a “yes” checkbox or “not empty” text field is complete.

Yes, this accounts for the fact that we have three objects "in play" -- Course, Activity, Event.
We also have a Lodge Stay object, but we have not yet develop a feedback survey for them. We do have a feedback idea for ti, so we will get to it at some point.

For your first question, yes. If we expand the logic, I think there's, OR (3) Both the check box is Yes and the text is not empty.
I'm not clear on your second question. If you are just referring to the variations of object type, then yes, all should behave the same.

I assume you are asking for one email to the safety committee if (1) the person answers Yes to "there was an incident" OR (2) the safety comments field is not empty. Is that correct?
Also, we have two similar field pairs on the course feedback form and the event feedback form. Do you want the same functionality on them too?
Note that this will require development and I am not sure how much until I ask. If it's relatively easy, I'll get it done in Apr. If harder, we'll see where it fits.
