Add Global Adventures Waiver to GA registration process
Make it so global adventure participants are prompted to fill out the global adventures waiver form when registering for the activity

We added Application, Trip Agreement, and Release Forms to used with Global Adventure activities.

It's probably much better to make this more like the "leader's permission/application form required" feature we use for courses, so that it could be used for any activity.
