Create More Robust Safety Committee Data Analysis SD52/465/466
Currently, participants submit incident reports to the Safety Committee, which then compiles the data and looks for trends in the aggregate (it does a lot more than that as well, but this aspect of its work is onerous). If we connected incident reporting with routes/places and even illustrated on a map, committees could analyze trends and information much more rapidly and wouldn't have to wait for Safety Committee reports. Safety Committee efforts could then be more available for training and prevention work now that the data crunching burden has been reduced. Fix incident report excel export bug
The incident report exports narratives in HTML. Change to plain text format. Example:

The way the report renders currently is not a bug. It is how rich text data is stored in the file of completed forms. Rich text allows those filing a report to be able to use simple formatting to add things like paragraph breaks in the narratives. Changing the Incident Narrative and Lessons Learned fields from Rich Text to Plain Text would be a huge step backwards, making it much harder to collect good data.
This rich text only affects the data file download. There are two other sources of submitted incident reports, (1) the emailed report submission, and (2) the stored report submissions. Both of these are nicely formatted and easy to read.
Because it’s raw data, it should be relatively easy to turn the downloaded file into one nicely-formatted document with any number of tools. One simple approach is to use the “mail merge” capability in the Word… -
Add Emergency Contacts to Committee Rosters & Course Rosters GH1836
Currently, emergency contacts can be viewed / printed for Activity Rosters, but not Committee Rosters or Course Rosters. This creates a shortcoming in safety preparation for course field trips, as there is not a reliable means of having emergency contact info available for students & instructors participating at field trips. Would seemingly be a relatively easy fix to make Committee & Course Rosters function similar to Activity Rosters.

Need Secondary Emergency Contact when Spouse on a Trip GH2828
The Member Profile allows you to provide an emergency contact. This works well, but if it is a spouse or partner, and both of you go on the same trip (as happened on a recent backpack I led), that contact is not useful. Leaders end up needing to request a second contact. (My co-leader wanted this from me; the problem hadn't occurred to me.) For such situations, it would be helpful to have a second contact in the system.
My suggestion is to provide the option to enter a second emergency contact and phone number, and where it is provided…

Need better lighting outside the Seattle Program Center ~Facilities
It's pretty dark on winter night events with a high number of car/pedestrian interfaces from significant parking on the west side of a very busy street, and lots of people activity at the Mountaineers and near by venues. A couple of strategically placed street lights would help a lot. Would probably also help reduce car break in's at the park to have better lighting.

Emergency contact person - automated emails
It might be nice when a trip happens (not a course or seminar, just a trip) that an automated email gets sent out to everyone's emergency contact person letting them know that they're going on a trip and what to do if they don't come home. If they don't live with each other, I bet nobody ever tells their emergency contact person they're going on a trip. I don't know what the official process is, but maybe the email could say "Sally Smith is going on a 1-day trip today with the Mountaineers, if they aren't home by noon tomorrow…

Email Incident Report Link Instead of Deatils GH3753
For increased security, instead of sending all the details of an incident report, send a link to the incident report that was submitted. The incident report link should be sent to:
- Safety Committee Admins
- Chair of sponsoring branch
- Chair of sponsoring committee
- Leader of activity
- Report submitter

Climbing leaders need a course in basic geology, see West Ridge Cuttroat report.
Mass wasting, the geological term, should not include Mountaineers.
When I used to guide and teach climbing professionally, I would ask my clients and students to consider how all those rocks in the scree and talus slopes got there. Great suggestion Eric. As a volunteer-run organization, the content of our courses is determined by volunteers. We’ve had a number of sessions on local geology offered in the past, and would certainly support more in the future (ex: https://www.mountaineers.org/locations-lodges/olympia-branch/committees/olympia-hiking-backpacking-committee/course-templates/geology-in-your-own-backyard-olympia/activity-templates/geology-in-your-own-back-yard). The best way to ensure this curriculum is included is to work with the climbing committee at your branch, and while we cannot require this of our climb leaders, we can certainly encourage them to learn more about the natural world in addition to their climbing and leadership skills training.

Activity, Course, Event Feedback: Email to Safety Committee SD378
As safety@mountaineers.org, I get an email when a participant answers "yes" to a "was there a safety concern or incident" question on a feedback survey about whether there was a safety concern on the activity, course or event. The participant's comments along with some meta data about the activity, course or event and the participant along with the participant's comments will be included in the email.

Store "Autopopulated" Data in Stored Incident Reports GH2705
When an Incident Report is submitted, we gather information from the activity, route/place, contact (leader), etc. to send by email and store in the data file. We also store the individual responses as content items and want the "autopopulated" info from the activity, route/place, etc. to be stored with each submission.

Prevent Deleting Incident Report Data and Incident Report GH2609
In Revise Save Data Adapter View, http://feedback.mountaineers.org/forums/273688-general-feedback/suggestions/37329700-revise-save-data-adapter-view-gh2941, we made it more difficult to empty the saved form data file, but for the Incident Report Form, we want to make it so that only Site Managers (the highest level of site admins) can delete this form or any per of it and hide the "Clear saved input" link from everyone but them.

Map of phone numbers to call for back country rescue
How about posting a map showing which number should be called in case an emergency rescue is needed in any given area in the Washington Cascades (or Olympics)? Maybe it already exists, but I couldn't find it. Ideally, the map would clearly delineate the boundaries between rescue jurisdictions.

Although there are some general jurisdictions for SAR teams they do work with eachother and the Navy, different county Sheriff’s office etc to see what resources are available most quickly. Most of the time the most useful information is to know whether you are in a National Park or if you aren’t in a National Park which county you are in along with the trailhead where you entered. Washington Department of Transportation has some good maps online to help identify which county you are in before heading out. http://www.wsdot.wa.gov/mapsdata/products/digitalmapsdata.htm
The next step after 911 is calling the Mountaineers specific emergency line 206-521-6030 and the folks on that line will additionally help triage the situation.

Improve Incident Reporting GH824
Incident reporting appears as just an embedded email link in the trip report
when a leader closes an activity. Its output are the emails that appear
from those trip leaders sending the email. That makes it harder for the
Safety Officer to identify and group them in mail. Basic metadata like
which branch this pertains to is not appended in the body. That makes it
hard to determine who is accountable for follow up. It would be really nice
if we could re-institute a form that writes to the DB and generates an email

Add to Check to Feedback Forms for Incident Report Info GH2731/2732
When checking whether to send an email to the Safety Committee (for all feedback forms) and whether to direct the user to the Incident Report Form (for activity-related feedback forms only) after submitting a Feedback Survey Form, in addition to checking if the answer to "Were there any safety incidents or near-misses on this trip? " is "yes," also check to see if the "Describe the incident or near-miss." response in NOT empty, If either is true send the Safety Committee and email and redirect the user to the Incident Report.
