Please note that all our courses, events, trips and course-related activities have feedback forms so that you give feedback for every activity in which you participate. You’ll get an email reminder to do so just after the activity. If your feedback is of urgent nature it may be best to contact the chair of the committee that sponsors that program directly. Not sure who the chair is? Contact our Member Services Team at info@mountaineers.org.

If there was a safety incident or near miss, you will have the opportunity for submitting an Incident Report after submitting activity feedback. You may also use the Incident Report button on the activity detail page (from your My Activities page) or contact our Safety Committee directly at safety@mountaineers.org.

If you reached this site wanting to report problem behavior by another member or harassment. Please submit a Behavioral Complaint Form.

  1. Add Emergency Contacts to Committee Rosters & Course Rosters GH1836

Currently, emergency contacts can be viewed / printed for Activity Rosters, but not Committee Rosters or Course Rosters. This creates a shortcoming in safety preparation for course field trips, as there is not a reliable means of having emergency contact info available for students & instructors participating at field trips. Would seemingly be a relatively easy fix to make Committee & Course Rosters function similar to Activity Rosters.

  2. Need better lighting outside the Seattle Program Center ~Facilities

It's pretty dark on winter night events with a high number of car/pedestrian interfaces from significant parking on the west side of a very busy street, and lots of people activity at the Mountaineers and near by venues. A couple of strategically placed street lights would help a lot. Would probably also help reduce car break in's at the park to have better lighting.

We are approaching this from several different angles.

    1. I am working with Diana Kinkaid, former Board member of Seattle Parks, to submit an application for a grant to fund design of safe pedestrian & automobile travel in the park. If the grant funds are secured Seattle DOT and Parks will develop a plan. Then there would be a subsequent process to gain funds to implement the design. This is a multi-year project.
    2. The Friction Slab project includes funds to construct an enclosure around the garbage and recycling containers in the south parking lot. A corner pole of the enclosure will be installed substantially higher than the others and lights will be installed at the top of this pole, improving lighting in the south parking lot. The Friction Slab project is on pause while we work through transitioning from one contractor to another contractor. Once beyond this pause, we…

  3. Create More Robust Safety Committee Data Analysis GH2704/2732/2705/2609/2491

Currently, participants submit incident reports to the Safety Committee, which then compiles the data and looks for trends in the aggregate (it does a lot more than that as well, but this aspect of its work is onerous). If we connected incident reporting with routes/places and even illustrated on a map, committees could analyze trends and information much more rapidly and wouldn't have to wait for Safety Committee reports. Safety Committee efforts could then be more available for training and prevention work now that the data crunching burden has been reduced. Members and Leaders could then have information about incident…

COMPLETED: Participants may now fill an incident report like the primary leader. This can be done from a button on the activity detail page or at the end the feedback survey.

ON HOLD: The Safety Committee completed changes to the Incident Report Form that we are implementing, but wants to re-review it before going live. The current estimate for launch is late 2018.

Please see the IT Planning & Prioritization sheet, https://docs.google.com/spreadsheets/d/1x1_tMuGpZZC4-xxyiDTzALvnbSPn2SJz3FveQQuyfmY/edit?usp=sharing, for where this project is ranked among other projects.

  4. Need Secondary Emergency Contact when Spouse on trip GH2828

The Member Profile allows you to provide an emergency contact. This works well, but if it is a spouse or partner, and both of you go on the same trip (as happened on a recent backpack I led), that contact is not useful. Leaders end up needing to request a second contact. (My co-leader wanted this from me; the problem hadn't occurred to me.) For such situations, it would be helpful to have a second contact in the system.

My suggestion is to provide the option to enter a second emergency contact and phone number, and where it is provided…

  5. Participant Note "Types"

    Participant Note Types:

Enhance participant notes capability so that users with the ability to add participant notes may select from a range of pre-determined “note types” (still to be determined, but could include “positive comment, future leader potential, conditioning issue, behavior issue, other comment”) when entering participant notes in addition to entering free text notes. Note types as well as free text become a part of a member’s profile.

    Filtering Note Types:

Users with the ability to research participant notes are able to filter participant notes by the pre-determined note types. This allows activity leaders and committee leaders to quickly…

  6. Upload incident data into salesforce

Connecting incident reports to flow into Salesforce would allow us to use reports and dashboards to identify trends in more robust ways. Is this possible for the future?

  7. Emergency contact person - automated emails

It might be nice when a trip happens (not a course or seminar, just a trip) that an automated email gets sent out to everyone's emergency contact person letting them know that they're going on a trip and what to do if they don't come home. If they don't live with each other, I bet nobody ever tells their emergency contact person they're going on a trip. I don't know what the official process is, but maybe the email could say "Sally Smith is going on a 1-day trip today with the Mountaineers, if they aren't home by noon tomorrow…

  8. Clean up the Safety Concern section of the Participant Survey form GH2731

    This request is two part

1.) Modify the label and description of the Y/N Safety Concern question

Label: "Were there any safety incidents or other concerns to report?"

    Description: [NO CHANGE]

2.) Add a text field to the Participant Survey to describe their safety concern.

    Label: "Please describe your concern"

Description: "Provide a short description of your safety concern"

    Content Type: Text field

Business Rules: Like #1 above, if content is entered into this text field, send safety report to Safety@mountaineers.org

User Story: As a trip participant, when I enter a description of my safety concern and click Submit to…

  9. Enable Safety Officers to view rosters and submit incident reports GH2744/SD668

Enable Safety Officers to perform the following
    1. Safety Officer access to activity, course and event rosters (GH2744--Complete)
    2. Safety Officer ability to submit an Incident Report tied to an activity, course or event (SD668--TBD)

Where Safety Officers are members of the Safety Committee roster and Leader status on that roster is True

  10. Map of phone numbers to call for back country rescue

How about posting a map showing which number should be called in case an emergency rescue is needed in any given area in the Washington Cascades (or Olympics)? Maybe it already exists, but I couldn't find it. Ideally, the map would clearly delineate the boundaries between rescue jurisdictions.

    Cancelled  ·  Garrett Arnold responded

As Jeff mentioned in the comments below, calling 911 is the very first thing that should be done in an emergency situation. Read more about our Emergency Contact Procedures here: https://www.mountaineers.org/about/safety/emergency-contact-procedures/view.

Although there are some general jurisdictions for SAR teams they do work with eachother and the Navy, different county Sheriff’s office etc to see what resources are available most quickly. Most of the time the most useful information is to know whether you are in a National Park or if you aren’t in a National Park which county you are in along with the trailhead where you entered. Washington Department of Transportation has some good maps online to help identify which county you are in before heading out. http://www.wsdot.wa.gov/mapsdata/products/digitalmapsdata.htm

The next step after 911 is calling the Mountaineers specific emergency line 206-521-6030 and the folks on that line will additionally help triage the situation.

  11. Improve Incident Reporting

Incident reporting appears as just an embedded email link in the trip report
when a leader closes an activity. Its output are the emails that appear
from those trip leaders sending the email. That makes it harder for the
Safety Officer to identify and group them in mail. Basic metadata like
which branch this pertains to is not appended in the body. That makes it
hard to determine who is accountable for follow up. It would be really nice
if we could re-institute a form that writes to the DB and generates an email
FROM a single incident reporting address…
