Update the Incident Report Form GH2704

Enable Safety Officers to Submit Incident Reports GH2964
Allow Safety Officer ability to submit an Incident Report tied to an activity, course or event on another person's behalf (e.g. from a phone interview).
3 votesThis idea has been incorporated into our Incident Report form already, so this idea is being marked as complete! As shown in the image attached, you can edit the name and email attached to an Incident Report to be yourself or someone else.

Activity, Course, Event Feedback: Email to Safety Committee SD378
As safety@mountaineers.org, I get an email when a participant answers "yes" to a "was there a safety concern or incident" question on a feedback survey about whether there was a safety concern on the activity, course or event. The participant's comments along with some meta data about the activity, course or event and the participant along with the participant's comments will be included in the email.

Allow Participants to Submit Incident Reports SD469
As an activity participant, I can report an incident, so that I can better communicate the details of an incident to the Safety Committee like the leader does.

Store "Autopopulated" Data in Stored Incident Reports GH2705
When an Incident Report is submitted, we gather information from the activity, route/place, contact (leader), etc. to send by email and store in the data file. We also store the individual responses as content items and want the "autopopulated" info from the activity, route/place, etc. to be stored with each submission.

Prevent Deleting Incident Report Data and Incident Report GH2609
In Revise Save Data Adapter View, http://feedback.mountaineers.org/forums/273688-general-feedback/suggestions/37329700-revise-save-data-adapter-view-gh2941, we made it more difficult to empty the saved form data file, but for the Incident Report Form, we want to make it so that only Site Managers (the highest level of site admins) can delete this form or any per of it and hide the "Clear saved input" link from everyone but them.

Need Secondary Emergency Contact when Spouse on a Trip
The Member Profile allows you to provide an emergency contact. This works well, but if it is a spouse or partner, and both of you go on the same trip (as happened on a recent backpack I led), that contact is not useful. Leaders end up needing to request a second contact. (My co-leader wanted this from me; the problem hadn't occurred to me.) For such situations, it would be helpful to have a second contact in the system.
My suggestion is to provide the option to enter a second emergency contact and phone number, and where it is provided…
7 votesThis idea's solution is now live on production! We now encourage member's to put multiple emergency contact numbers in the Emergency Contact Name and Number fields. This automatically flows through to all rosters and should cover if someone is on a trip with one of their emergency contacts. When members are prompted to resign their waiver, they will see the follow fields.

Clean up the Safety Concern section of the Participant Survey form GH2731
This request is two part
1.) Modify the label and description of the Y/N Safety Concern question
Label: "Were there any safety incidents or other concerns to report?"
Description: [NO CHANGE]
2.) Add a text field to the Participant Survey to describe their safety concern.
Label: "Please describe your concern"
Description: "Provide a short description of your safety concern"
Content Type: Text field
Business Rules: Like #1 above, if content is entered into this text field, send safety report to Safety@mountaineers.org
User Story: As a trip participant, when I enter a description of my safety concern and click Submit to…

Add Emergency Contacts to Committee Rosters & Course Rosters GH1836
Currently, emergency contacts can be viewed / printed for Activity Rosters, but not Committee Rosters or Course Rosters. This creates a shortcoming in safety preparation for course field trips, as there is not a reliable means of having emergency contact info available for students & instructors participating at field trips. Would seemingly be a relatively easy fix to make Committee & Course Rosters function similar to Activity Rosters.

Improve Incident Reporting GH824
Incident reporting appears as just an embedded email link in the trip report
when a leader closes an activity. Its output are the emails that appear
from those trip leaders sending the email. That makes it harder for the
Safety Officer to identify and group them in mail. Basic metadata like
which branch this pertains to is not appended in the body. That makes it
hard to determine who is accountable for follow up. It would be really nice
if we could re-institute a form that writes to the DB and generates an email

Add to Check to Feedback Forms for Incident Report Info GH2731/2732
When checking whether to send an email to the Safety Committee (for all feedback forms) and whether to direct the user to the Incident Report Form (for activity-related feedback forms only) after submitting a Feedback Survey Form, in addition to checking if the answer to "Were there any safety incidents or near-misses on this trip? " is "yes," also check to see if the "Describe the incident or near-miss." response in NOT empty, If either is true send the Safety Committee and email and redirect the user to the Incident Report.
